Opportunity Description:
The following states are eligible to apply: Arizona, California, Florida, Georgia, Illinois, Kentucky, Michigan, Missouri, New Jersey, New York, North Carolina, Ohio, Pennsylvania, and Texas.
The CVS Health Foundation is requesting grant proposals from local multi-sector partnerships seeking to improve older adult health. CVS Health Foundation will award five multi-year, $1 million grants through its Focus on Healthy Aging (FOHA) program. FOHA will support mature collective impact efforts that strengthen systems-of-care for older adults with brain, heart, and mental health conditions.
