Opportunity Description:

DAC Healthcare System Preparedness (DAC-SP) is seeking applications from U.S. healthcare systems interested in implementing and evaluating the Early Detection Blueprint for Cognitive Impairment as a clinical workflow improvement project. Up to 10 U.S. healthcare systems will be selected for this fellowship program based on their demonstrated readiness and capacity for implementing the blueprint. DAC-SP will consider a broad range of criteria in site selection to help ensure the diversity of participating healthcare systems, including geographic regions, health system type, size, and populations served.
 
If selected, participants will be provided grant funding and technical support, as well as join a Community of Practice to extend their national leadership in the field while helping transform their local clinical practices. The U.S. Fellowship Program is part of DAC-SP's mission to catalyze healthcare system transformation and to turn research breakthroughs into lasting improvements in clinical practice.
