Supporting Documents
Please Note:
In order for your application to be complete you need to also submit the following documents to
mjsamuel@americangeriatrics.org by October 1, 2018:
- Two letters of support from current AGS Board and/or Committee members
- A 2 page biographical sketch or short curriculum vitae.
If the person is being nominated by a current or past AGS Board Member, a completed ‘AGS Board of Directors Nomination Form’ can be used as their letter of support. If not, AGS staff will follow-up with the nominee, and ask them to obtain a letter of support in order to complete their application.
